With my RT-N66U's stock firmware, I can plug in a 4g hotspot device via USB cable and the router can use it as the wan/uplink. This is not the case with TomatoUSB. How can I build / enable USB Networking support for TomatoUSB?

Which build of TomatoUSB are you using?

Try a Toastman or Shibby flavor of ToamtoUSB.

I am using Shibby's build. USB Network does not work. 3g modem devices that direct plug to usb work, not actual usb network devices like hotspots or phones.

It's no more a network device than a 3g modem. To the router it's just a USB device. An unknown USB device that it doesn't have a driver for.

What probably needs to happen is an update to the module that's providing USB 3g modem services to support new USB devices. Assuming that module has been updated to support them (big supposition on my part).

Hopefully shibby sees this, since you're using his build he's probably the guy in the know. I don't remember which build was first to have 3g modem support...
