I have a WRT54gs V4 running the latest thibor firmware.

Do I need to change any settings for bittorrent to run optimally?

For example, manually setting port forwarding?

I see in the admin page that a uPNP port forwarding is automaticallly setup when bittorrent starts.

If your BitTorrent client supports UPnP, then you don't need to do it manually. No other settings changes are required, though you can play with the "conntrack" settings on the Admin page, which control how quickly idle connections get timed out and recycled, and how many connections to maintain at one time.
